If you need a refill and it has been 6 months or more since your last follow up visit, please book a follow up visit with Dr. Rahr at www.drnatalierahr.com. No refund is given if this request is paid for and it has been 6 months or longer. A requested change mto a recent prescription may require a follow up visit.

 

Please include the prescription name, dose, pharmacy name and contact number. As well as any other specific instructions.

 

Prescription must be for an ongoing treatment that patient and Dr. Rahr have discussed in a previous visit. Dr. Rahr reserves the right to refuse or change or place conditions on a refill request. Requests made for medications that are not current ongoing treatment, or that are for medications tried in the past and were discontinued or were meant to be temporary, will require a visit booked to discuss before a refill is granted. Refill is not guaranteed and fee will not be reimbursed if paid.
